If medication is to be given to your child during school hours, parents/guardians must follow the procedure as outlined below:
- A physician must complete the Prescription Medication Form for all prescription medications or any over-the-counter medications that will be kept in the clinic for greater than five (5) school days.
- Parents or guardians may complete an Over-the-Counter form for any over-the-counter medication that will only be given for five (5) school days, in grades K-5 and Grades 6-8. In Grades 9-12, parents may complete the Over-the-Counter section in the First Day Forms, allowing their child to carry and self-administer the listed OTC medications.
- A parent or guardian must deliver medications to the clinic for grades K - 8th. No student may transport medication on the bus either to or from school. If a student is found to be carrying medication on their person or if medication is delivered to the clinic by a student, said student may be subject to disciplinary action.
