Health Concerns & Medication Information (2019-2020 School Year)

Does your child have a health concern that requires medical attention while at school?

  • asthma
  • seizures
  • diabetes
  • allergic reactions (food, bees, etc.)
  • other health impairments requiring daily or emergency treatments

Does your child require medications while at school?

  • ADHD medication
  • migraine medication
  • medication for medical conditions taken on a regular basis

If you answered YES to the above questions, please click on this link: Health Forms.  Please complete the appropriate forms, have your child's healthcare provider sign them and return them to the designated building nurse before the first day of school.
