2025-2026 ATHLETIC REGISTRATIONJackson Local Schools continues its partnership with Final Forms to complete and sign Athletic Forms for your student-athletes.  

  • If you are a returning Athletic Family then you will receive an email from Final Forms in mid-June to sign into your account to register for 25-26.  
  • If you did not compete for us last year then you can start the process at jacksonlocal-oh.finalforms.com.  
  • All student-athletes MUST have their Athletic Registration completed and their 25-26 Sports Physical uploaded to Final Forms BEFORE the official OHSAA first day in order to participate with their sport on the OHSAA official tryouts/practices dates.  Student-athletes cannot participate in tryouts or practice without this step done. 

Physicals must be uploaded to Final Forms.  Physicals will NOT be accepted at JHS/JMMS.

Do not give any of your paperwork or Athletic fees to a coach.

Pay to Participate Athletic Fee

Pay your athletes Pay to Participate fee through your Final Forms account (Middle School fee is $75 per season/per athlete,  Family cap $300)

Payments Due:  Fall (Sept 6th)    Winter (Jan 6th)     Spring (April 30th)

This year we have athletic ticket purchase options.

You can pay cash at the gate or if on-line ticketing is your option then the Jackson Ticketing Box Office can be accessed through: Click here for tickets

Individual game tickets will not be available until one week prior to the contest.  Suggest not purchasing on-line tickets until the day of the event in case of any cancellations. You can Purchase team season passes here as well.

  • Senior Citizen All Year Sports Pass 

Purchase through Jackson Ticketing Box Office

  • Jackson Student All Year Sports Pass  - $75  - good for all HS and MS events

Individual MS sports like Volleyball and Basketball will have Season Passes with the multiple number of home games for each team.  These will also be available at the beginning of each respective season through our Jackson Ticketing Box Office.  High School also has individual season passes for most sports as well.
