*** COVID-19 VACCINATION CLINIC OFFERED MAY 27 ***
The Stark County Health Department will provide a school-based, voluntary Pfizer COVID-19 vaccination clinic at Jackson High School on May 27 from 10 a.m. to 2 p.m. and 3 p.m. to 5:30 p.m. This vaccination clinic is for all students 12 years and older. Parents wishing to receive a vaccine may do so as well. Students will enter Door 18W and exit Door 17W (see map below). Please keep in mind, there is a 15-minute observation period.
Parents are permitted to accompany their child(ren) inside the clinic. However, those parents wishing to wait outside may do so as well. Parents who will not be with their child(ren) will have to sign the attached permission form from the Stark County Health Department and have their child(ren) bring it to the clinic.
There is no out-of-pocket expense to families for this voluntary vaccination. The Stark County Health Department will bill an administration fee to insurance carriers for those students covered by their parent(s)’ coverage.
IMPORTANT: All those who wish to receive the vaccine MUST complete the registration process by clicking on Invitation Web Link. Please read the attachment from the Stark County Health Department.
The Jackson Local School District is working collaboratively with the Stark County Health Department to provide this voluntary vaccination clinic. The district and health department are working together to provide a comprehensive vaccination clinic in August before the start of school. More information on details of that clinic will be provided at a later date.
